Document Services

SVA offers a complete document package, including all necessary administrative forms. Once a plan is selected, we will obtain IRS approval, if needed. The document package may include:

  • Plan adoption agreement and trust
  • Notice to employees
  • Summary plan description
  • Administration forms (Designation of Beneficiary, Salary Deferral Agreement, etc...)
  • Determination letter applications
Tax Reporting
